Chronic Disease Management

Access Health (SA) established itself as a chronic disease management entity as early as 1996. Our disease management programmes are lifestyle oriented with intensive education, support and compliance management as the major pillars of the programmes. The programme is referred to as the access lifestyle™ programme.

Our disease management programmes are excellent solutions within managed healthcare as well as employee wellness contexts. Since 1996 we have stayed the leader in disease management and focus on reducing risk and improving compliance.

Our comprehensive lifestyle related Chronic Disease Management Programmes assist our clients in the following clinical conditions:

  • Hypertension
  • Cardiac conditions
  • Asthma
  • Hypercholesterolemia
  • Diabetes
  • Mental problems such as depression and anxiety
  • HIV / AIDS

The WARS IndexTM forms the backbone of the outcomes from the chronic disease management programmes and is totally risk oriented. Reporting and outcomes measurement would then also focus on change in risk behaviour and reducing risk categories. Our programmes ultimately integrate a medical model approach (essential for the clinical management aspects) as well as a psycho-social and behavioural model.

Programme Philosopy

The “one-umbrella” or flexible “plug-and-play” range of services included in the offering ensures that the people requirements are aligned with business requirements in the organisation. We can therefore offer the total comprehensive and integrated service to an employee base or we could render specific components complimentary to structures already in place, e.g. benefits of employees who are members of medical schemes and also should there be well established occupational health services in the company.
